Medicaid Service Coordinator

ILA is seeking a Medicaid Service Coordinator reporting to the MSC Program Coordinator. The candidate is defined as a person who works in partnership with an Individual and, if desired, his/her advocate, creating and maintaining an Individualized Service Plan (ISP).

The focus for each person’s ISP is a written plan of services which identifies activities, supports and services that will assist the Individual to live and participate in the community consistent with his or her preferences and needs. The MSC works with individual to enable them to live and participate in the community according to their personal goals, preferences, and needs.
